...via Burwood Highway:
Take the Burwood Highway to Belgrave. At the end of the shopping strip, turn right at the roundabout into the Belgrave - Hallam Road and, a few seconds later turn left at the roundabout along the Belgrave - Gembrook Road towards Emerald — if you are lucky, you'll see Puffing Billy as you drive under the trestle bridge. Enjoy the spectacular views until you see St Cuthbert's on the corner of Railway Parade on your right, straight after Church Road....

...via Wellington Road 
I usually take the Monash Freeway to Wellington Road when coming from the city):
Drive eastwards along Wellington Road for about 20 minutes from the Monash Freeway, enjoy the drive but watch the bends until you get to the T-intersection at the very end (opposite the Paradise Hotel). Turn left and, after two minutes, look out for St Cuthbert's on the corner of Railway Parade on your left.

...via Train
Belgrave line train (Connex Trains) to Belgrave Station.  There is a short walk down the platform to Puffing Billy (Narrow gauge century-old steam train) which you take to Menzies Creek station.  Follow map for directions for the 5 minute walk to the chapel.
